Student Confidential and Image Consent Forms
Have any of your details changed? Have you moved house or changed your contact phone number/s? Do you have a new person who you wish to add as an emergency contact during Academy hours for your child? You may have changed GP surgeries or want to inform us of new medical conditions.
If any details change we ask that you inform us so our records are accurate and up-to-date. If you have any confidential changes to tell us about please complete the form and return it to the Academy as soon as possible.

Have you completed an Image Consent Form?
This form allows you to choose whether we as an Academy can use images of your child for various purposes. We will not use any images without your consent. If you change your mind you must inform us in writing or complete another form and return it to the Academy.
